So here is the explanation again:

But it seems that this drama has really a different approach: I have been always wondering why this drama was called the descendants of the sun. But now, after this episode I believe, I've got the answer because it is clear that the romance isn't the main topic, rather the lives of doctors and soldiers (especially from the UN who have a different function than national soldiers) in devastated countries (earthquake or civil war). Sun is represented in the Greek mythology by Apollo. This god is associated with medicine and healing, but also with ill-health and plague. We all know that the sun symbolises hope and care too. So YSJ and MY as soldiers and doctors represent this god: the two faces, healer and bringer of disease. Even as a doctor, you can't always save lives.

Then Apollo also protects too like the soldier YSJ. So YSJ and KMY are his "children" because they are acting like him.

Sun represents also life too: without the sun, life would be impossible that's why KMY and YSJ are representants of the sun because they can bring life back (after a disaster or a civil war).

O my, lots of feels and so much tears, this episode and looking at preview, it will get worse today! Kudos to all crew and cast - backdrop was simple and when disaster struck, props were simple too but gosh, even that simple scene of handsome doctor putting back up the clock with his pretty wife made me cry! That's the reality, people die but lives move on and time waits for no one. No, not even our two lovebirds - I hope MY makes up her mind soon and we'd have at least some happy times and more kisses before Captain Argus, the bad guy appears again It's almost half way through the drama but already I'm dreading the time when drama ends - I know I will miss it like crazy!!

This is probably an unpopular opinion, at least judging by the posts I've seen discussing the topic, but to me, Shi-jin's words to Mo-yeon while she was stitching him up had a different meaning. If we take the conversation as kindly translated by @miri13:

I am choosing this variation of the translation because I too agree that 'wanting to see her' suits better his honest-to-a-fault personality. But I digress, the point I'm trying to make is that their conversation had three different moods: at the start it was awkward as Shi-jin tried to comfort her using a style that's clearly not his usual one, she called him out on that and asked him to crack a joke, which he did succeding in making her smile. He then lowered his tone of voice and, almost as though as he was talking to himself, he did not confess, what I feel he did was admit defeat. He was basically telling her (and himself) that he had lost the battle against his own will and that no matter what his best intentions were, he simply could not forget her. Another reason I don't think it was a confession is because, unlike the previous time, he did not ask for any feedback. We've seen the way he acts when he wants a response, like for instance the first time he was whisked away by helicopter, he was almost forceful in getting an answer out of her.

The same thing happened while he asked whether she wanted him to apologise or confess. He wanted an immediate answer. This time though, he's not asking anything from her. All he wants is for her to think about the fact that he'll never be able to forget her no matter what. No feedback is required. He's already conceeded defeat and just wanted her to know it too. And about being serious (or conveying her that this are his sincere thoughts), it doesn't mean he wasn't serious before, he was incredibly serious about them pretty much from the moment he got the first inkling she had feelings for him too. I feel he told her he was serious because, up to that moment, she had asked him to joke and he wanted to be sure she knew that this part of the conversation was not one of his usual jokes. Last but not least, I would not be surprised if at the start of today's episode, he just gets up, thanks her for taking care of his wound and leaves. After all, I think that he understands now that he's made all the moves he could make, and even if deep down he hopes it, he's not all that confident that she'll want to make any move at all.
