Gun specifically told Grandma he wanted to wait to have kids, and enjoy the honeymoon period with Mi Young. I really dislike that she did not respect that. 
I don't think they have sex immediately after Gun enters the room. The drug takes time to kick in. Also, between that scene and the rice-barley game, the show inserts a scene of Grandma and Yong's mother talking to the ancestors. If they make love immediately, the rice-barley game would be shown right after Gun hugs Mi Young in bed.
