Hyperpigmented areas actually naturally goes away on its own with time. You could however speed up his process through exfoliation. Exfoliation helps with removal of hyperpigmented dead skin cells which helps with the discolouration. I would recommend using a chemical exfoliant such as AHA which can be found in certain skincare products. Try to avoid using a physical exfoliant (e.g. scrubs) as many of these products are actually too harsh for your skin and do more damage than good.
Another tip is to use sunscreen with a good SPF or PA which prevents your hyperpigmentation from getting darker through exposure to UV rays. Remember to wear it even on cloudy days as UVA penetrates through clouds and smog.
Of course you can always go see a dermatologist and a peeling done professionally.

The most effective way, without trying out a bunch of products, is by going to a dermatologist and ask her/him to prescribe Retinol cream.
Or go to a facial place, the specialist will also recommend Retinol treatment or chemical peel. I want this treatment to remove my freckles/sun damaged face, but because of some health-related issue, I can't have it. :(

The Vichy Proeven day and night creams really helped me with my hyper-pigmentaton and post-acne scars. I also try to wear sunscreen everyday and exfoliate regularly.

I just replied to another thread about acne scars, this same procedure will help with hyper-pigmentation as well. I wouldn't advise using any lightening creams since they make you more sensitive to the sun and will make you more prone to damage and further hyper-pigmentation, but if I have to recommend a product that is more natural and works it's Le Mieux Brightening Serum. ALWAYS use suncresen with it (and any other product). You will get more pigmentation if you go in the sun after using these products. They rip off essential layers of your skin that protect you. That's why Dermpapen is the best, it uses your own body to heal your skin. To quote my other reply:

Dermapen is amazing. I suffered from acne for a decade and still get breakouts now. I had enlarged pores and scars and after a couple sessions I saw a good improvement. I'm on my fifth now, and my skin looks like it did before I started getting acne (like 8 lol). It's expensive, but so worth it. It works for fine wrinkles and uneven skin tone.

Essentially it uses needles to purposefully damage your skin just enough that your body produces its own collagen to heal. Lasers and stuff I'm scared off because it takes off your skin and makes it so sensitive to the sun and other products, but Dermapen is making your body produce its own healing product! Your face will be red for the first three days and it will start to peel, but then you will look soooo fresh after. 

Google before and after Dermapen and you'll see what happens. It's no BS.

My pores were enlarged & acne scars left after I recover from a really bad Acne. Had tried products on the market , home remedy , from dermatologist but nothing seems to show really visible results. And I'm so scared I'll screwed up with my face again.

Usually hyperpigmentation will fade over time but depending on the person the time and speed of the process will differ, you can use different products which will help in the process
some natural remedies are lemon juice, you apply it for 10 minutes and wash it off because it's too harsh to keep on your skin for too long
there are also loads of 'whitening' products out in the market especially if you live in Asia, don't be scared of the word 'whitening' it potentially only means 'brightening', unless you use it over a long period of time your face will not have dramatic whitening effect
ones I recommend are the Laneige white plus renew line, this really helped me with my hyperpigmentation fade much quicker and making my skin look brighter and more translucent overall
another product I recommend for stubborn on scars, say you only have one or two scars on your cheek is to apply organic rosehip oil with a q-tip on the scar
